ABSTRACT

Abstract: Background: Basal cell carcinoma is the most common type of skin cancer. Although the literature provides a great deal of information on the recurrences of basal cell carcinoma, studies about these indices addressing only the cases in which flaps and/or grafts have been performed for surgical reconstruction of the excision of this tumor are still lacking. Objectives: To evaluate rates of recurrence of basal cell carcinoma submitted to conventional surgery with pre-established margins and reconstruction by flaps or grafts. Methods: A retrospective and observational study was performed through the analysis of 109 patients, who met inclusion criteria with 116 basal cell carcinomas submitted to conventional surgery and pre-established safety margins, requiring reconstruction through a graft or cutaneous flap. This work was performed the small surgeries sector of Dermatology of the Specialty Outpatient Clinic of the University Hospital of the State University of Londrina, between January 1, 2011 and December 31, 2015. The following data were collected and inserted in an Excel worksheet: name, registration number of the hospital patient, sex, age, tumor location, histopathological type of BCC, procedure performed (type of flap and/or graft), follow-up time, recurrence. Results: Of the 116 procedures, there were recurrences in 3 cases (2.6%) that were located in the nasal region and related to sclerodermiform or micronodular histological types. Study limitations: Retrospective nature of the study. Conclusion: The present study of the dermatology department of this university hospital showed a low rate of recurrence of basal cell carcinoma in cases where flaps and/or grafts were used in the surgical reconstruction.

ABSTRACT

Introdução: Estudos recentes apontam a utilização do curativo biológico com base em animais aquáticos como biomaterial na medicina regenerativa, apresentando boa aderência ao leito das feridas. O objetivo foi avaliar a eficácia da utilização da pele da Tilápia-do-Nilo (Oreochromis niloticus) como curativo biológico oclusivo, no manejo/tratamento de queimaduras de 2º grau em adultos. Métodos: Estudo clínico com 30 pacientes aleatoriamente tratados com pele da Tilápia-do-Nilo (n = 15) e hidrofibra com prata Aquacel Ag® (n =1 5). Resultados: Em relação à duração, o tratamento com a pele da Tilápiado-Nilo obteve uma média de dias de tratamento (9,6 ± 2,4) similar ao material comparativo (10,7 ± 4,5). Quanto ao relato de dor durante a troca de curativos, não houve diferença estatisticamente significante (p > 0,68) entre os grupos. Após a troca do curativo, não houve inferioridade no registro do valor na escala analógica de dor, em que 66,7% dos tratados com pele da Tilápia-do-Nilo relataram diminuição dos eventos álgicos. Constatou-se ainda que 60% dos pacientes tratados com a pele da Tilápia-do-Nilo não tiveram seus curativos substituídos em qualquer momento do tratamento. Para o curativo Aquacel AG®, 53,3% dos pacientes tiveram mais de uma substituição de curativos. Conclusões: Com base na pesquisa, pode-se concluir que a pele da Tilápia-do-Nilo é eficaz como curativo biológico oclusivo. Houve similaridade entre os grupos para a média de dias de tratamento (completa cicatrização da ferida) e para o relato de dor durante a realização do curativo. Também, a não inferioridade relacionada a dor após os curativos e suas trocas (quando existentes) e na quantidade de substituições destes.


Introduction: Recent studies have suggested the use of biological dressings made of aquatic animals as biomaterials in regenerative medicine since they demonstrate good adherence to the wound bed. The objective of this study was to evaluate the efficacy of Nile tilapia skin (Oreochromis niloticus) as an occlusive biological dressing in the management and treatment of second-degree burns in adults. Methods: This clinical study included 30 patients randomly treated with Nile tilapia skin (n = 15) or Aquacel Ag® silver-based hydrofiber dressing (n = 15). Results: The Nile tilapia skin yielded a similar mean treatment time (9.6 ± 2.4 days) to that of the comparative material (10.7 ± 4.5 days). There was no statistically significant intergroup difference (p > 0.68) in pain during dressing changes. No disadvantage in pain was noted, as 66.7% of patients treated with Nile Tilapia skin reported a decrease in pain events. Moreover, 60% of the patients treated with the Nile Tilapia skin did not require dressing replacement at any time during treatment. For the Aquacel AG® dressing, 53.3% of the patients required more than one dressing replacement. Conclusions: Our findings suggest that the Nile tilapia skin is as effective as an occlusive biological dressing. The average treatment time (complete wound healing) and pain reports during dressing changes were similar between groups. Furthermore, pain after and number of dressing exchanges (when performed) were not worse.

ABSTRACT

INTRODUÇÃO: O aumento da sobrevida na fase aguda do paciente grande queimado faz também aumentar a prevalência das sequelas cicatriciais estético-funcionais. Ao pensarmos na relevância clínica da lipoenxertia subcicatricial, necessita-se da compreensão e avaliação microscópica das reais alterações após o procedimento. MÉTODOS: Foram selecionados oito pacientes, vítimas de queimadura por álcool, com tempo médio após queimadura de 12 meses (10-14 meses), sendo submetidos à lipoenxertia subcicatricial associada à Rigottomy. Foram realizadas biópsias cutâneas no pré e pós-operatório tardio com 14 semanas. Avaliados os quesitos: 1 - Melhora estético funcional da cicatriz pela escala de Vancouver; 2 - Análise quantitativa e qualitativa do colágeno cicatricial; 3 -Análise imunohistoquímica da vascularização cicatricial com anti-fator de crescimento derivado do endotélio vascular (antiVEGE). RESULTADOS: Ao compararmos o período pré e pós-lipoenxertia, pôde-se avaliar que houve melhora estético-funcional significativa e, microscopicamente, redefinição entre os limites da derme papilar e reticular; redução quantitativa e reorganização do colágeno, além do decréscimo da vascularização tecidual pela análise imunohistoquímica. CONCLUSÃO: O princípio básico de todo processo cicatricial fisiológico é reestabelecer a homeostasia local, ou seja, as etapas exageradamente intensificadas levam a alterações clínicas catastróficas. Ao realizar a lipoenxertia subcicatricial associada à Rigotomia, foi verificada, neste estudo, a melhora qualitativa e quantitativa do tecido. Sendo assim, torna-se evidente o futuro promissor deste procedimento para a complementação terapêutica das patologias cicatriciais.


INTRODUCTION: The increase in survival of large burn patients during the acute phase has also increased the prevalence of esthetic-functional scarring sequelae. With regard to the clinical relevance of subcicatricial fat grafting, the actual changes after the procedure need to be understood and microscopically evaluated. METHODS: Eight patients with alcohol burns, with an average time after burn of 12 months (10-14 months), who underwent subcicatricial fat grafting associated with rigotomy were selected. Skin biopsies were performed in before and 14 weeks after operation. The following issues were assessed: 1) esthetic-functional improvement of the scar, by using the Vancouver scar scale; 2) quantitative and qualitative analyses of cicatricial collagen; 3) immunohistochemical analysis of scar vascularization with anti-vascular endothelial growth factor (anti-VEGF) antibody. RESULTS: When comparing the pre- and post-fat grafting period, a significant esthetic-functional improvement and, microscopically, a redefinition of the boundaries of the papillary and reticular dermis were observed, as well as quantitative reduction and reorganization of collagen, in addition to the decrease of the vascularization of the tissue through immunohistochemical analysis. CONCLUSION: The basic principle of the whole physiological healing process is to reestablish local homeostasis, that is, excessively intensified steps that lead to severe clinical changes. When subcicatricial fat grafting associated with rigotomy was performed, qualitative and quantitative improvements of the tissue were verified in this study. Thus, it becomes evident that this procedure can complement the treatment of cicatricial pathologies.

Introdução: O sucesso de um enxerto de pele é avaliado não apenas pela integração do enxerto em si, mas também pela qualidade da recuperação da área doadora. A despeito de as áreas doadoras de enxertos representarem o melhor local para estudo de cicatrização de feridas, regimes de tratamento, de áreas doadoras, tem sido incompletamente estudados. Objetivo: Avaliação da eficácia de espumas de poliuretano como curativo de áreas doadoras de enxertos. Método: Estudo prospectivo no qual áreas doadoras de enxertos foram tratadas com espumas de poliuretano como alternativa a filmes de acetato de celulose. Resultados: Foram tratados 11 pacientes e catorze áreas doadoras de enxerto. Aderência prolongada (73%) e odor desagradável (45%) foram os problemas encontrados. Os resultados foram considerados insatisfatórios na grande maioria dos casos (73%). Conclusões: O uso de espumas de poliuretano mostrou-se ineficaz, nesse grupo de pacientes, devido à ocorrência de alto índice de complicações.


Introduction: The success of a skin graft is evaluated by not only the integration of the graft itself, but also the quality of the recovery of the donor site. Despite the fact that graft donor sites represent the best place to study wound healing, treatment regimens for donor sites have not been studied extensively. Method: To evaluate the efficiency of polyurethane foam as a dressing for graft donor sites. Methods: We conducted a prospective study in which graft donor sites were treated with polyurethane foam dressing, as an alternative to a cellulose acetate film. Results: We treated 11 patients and 14 donor graft sites. Problems associated with the use of polyurethane foam included prolonged adherence (73%) and an unpleasant odor (45%). The majority of patients reported that they found the dressing to be unsatisfactory (73%). Conclusions: The use of a polyurethane foam was shown to be ineffective as a graft donor site dressing, due to the high rate of associated complications.

ABSTRACT

O pioderma gangrenoso é doença cutânea inflamatória rara, idiopática. Afeta principalmente adultos; apenas cerca de 4 por cento dos casos são diagnosticados em crianças e adolescentes. Existem quatro formas clínicas de pioderma gangrenoso: ulcerativa, pustular, bolhosa e vegetante (pioderma granulomatoso superficial). O pioderma granulomatoso superficial é considerado a forma mais benigna e incomum da doença. Em pacientes submetidos a manipulação cirúrgica, uma eventual manifestação do pioderma gangrenoso ocorre nos locais de intervenção. Relata-se o caso de criança de cinco anos de idade, vítima de queimadura, que apresentou pioderma granulomatoso superficial sobre áreas doadoras de enxertos.


Pyoderma gangrenosum is a rare idiopathic skin disease. It affects mainly adults, and only 4 percent of the cases are diagnosed on children and adolescents. There are four clinical forms of pyoderma gangrenosum: ulcerative, pustular, bullous, and vegetative (superficial granulomatous pyoderma). Superficial granulomatous pyoderma is considered the most benign and uncommon form of the disease. Patients who have undergone surgical procedures may occasionally present pyoderma gangrenosum manifestations on the surgical site. A case of a five-year-old child, victim of burn, who presented superficial granulomatous pyoderma on the skin graft donor sites is reported.

ABSTRACT

BACKGROUND: Suction blister grafting is a useful modality of treatment of patients with resistant and stable vitiligo. However, there have been no detailed studies to find out the best donor site for blister formation. METHODS: The study was conducted between the period of October 2004 and February 2005 in the dermatology department at a tertiary care center. Nine patients with vitiligo (focal vitiligo, 3; mucosal vitiligo, 2; acrofacial vitiligo, 2; vitiligo vulgaris, 1; and segmental vitiligo, 1) were selected for blister harvesting and grafting. The blisters were raised using the method described by Gupta et al. RESULTS: Suction blisters were attempted to be raised at 52 sites, but only 38 blisters could be raised, 24 complete and 14 incomplete. Blisters were raised in all the three cases on the flexor aspect of the arm (100%), 15 of 17 cases (88.2%) on the flexor aspect of the forearm, 4 of 5 cases (80%) on the abdomen, 11 of 16 cases (68.7%) on the anterolateral thigh, and less frequently over leg or foot. Complete blisters were formed in 13/15 cases (86.6%) on the flexor aspect of the forearm, 6/11 cases (54.5%) on the anterolateral thigh, and in all cases over leg. CONCLUSION: The flexor aspect of the forearm is a good site for suction blister harvesting.

ABSTRACT

A fixação dos enxertos de pele total através da sutura demanda tempo cirúrgico relativamente longo. O surgimento de adesivos teciduais tem se apresentado com várias aplicações clínicas, constituindo alternativa à sutura convencional. O objetivo deste estudo é avaliar a eficácia do adesivo 2-octilcianoacrilato, quando comparado à sutura simples, na fixação de enxertos de pele total, analisando-se a integração do enxerto. Foram retirados enxertos de pele total de 1 cm², bilateralmente, do dorso de nove ratos machos, com peso médio de 150 gramas. A pele retirada para a enxertia foi recolocada sobre o defeito, a fixação foi realizada através de sutura contínua com fio monofilamentar de náilon 4-0 no lado esquerdo e adesivo de 2-octilcianoacrilato no lado direito. Os aspectos clínico e histológico da integração do enxerto foram analisados no 7º e 14º dia de pós operatório. Utilizou-se o software Sens-a-Ray para comparação de resultados. Evidenciamos maior integração do enxerto de pele total no lado do adesivo de 2-octilcianoacrilato (80 por cento) em comparação ao lado da sutura contínua (44 por cento), sendo que houve perda total do enxerto do lado da cola de 2-octilcianoacrilato em 11,1 por cento do casos comparado a 33,3 por cento da sutura contínua. O tempo médio da fixação do enxerto no grupo da cola de 2-octilcianoacrilato foi de 23 segundos, contra 4 minutos da sutura convencional. Foi demonstrado uma maior integração e um menor tempo de fixação do enxerto no grupo do adesivo 2- octilcianoacrilato.

ABSTRACT

Tubularised flaps from the preputial, penile and scrotal skin were used for repair of severe degrees of hypospadius either as a primary one stage repair or as secondary repair of failed or compromised primary reconstruction in 28 boys. Primary one stage repair was done in [12] cases [5 of them were wrongly circumcised with deficient preputial skin]. Secondary reconstruction was perfomed in [16] cases after failed or complicated primary repair. Vertical flaps from the lateral penile shaft skin or the scrotum were utilised for urethral repair when the prepuce is lacking by previous circumcision or by previous repair. Scrotal skin can further be mobilised to cover penile shaft skin defects. Both penile and scrotal skin are suitable and available substitutes for hypospadiac urethral reconstruction when the prepuce is deficient or insufficient

ABSTRACT

Se intentió desarrolar un modelo experimental en eclampsia para estudiar su posible etiologia. Veinitdós ratas hembras de la cepa endocriada espontáneamente hipertensa (SHR) fueron injertadas con piel de machos de la cepa Holtzman. Se realizaron 4 injertos con intervalos de diez días. Cada SHR fue apareado con el macho donante correspondiente diez días después del último injerto. Como controles use usaron cinco SHR apareados con machos Holtzman si injertos previos. En la mayoría de las ratas experimentales que quedaron preñadas se encontraron alteraciones que consistieron en: bajo número de crías, fetos muertos, abortos y retardo dei crescimiento. La función renal fue evaluada antes y durante la preñez, mostrando un aumento fisiológico del filtrado glomerular del 7,5%, mientras que en la preñez normal se encotró un incremento del 166%. La histología renal mostró lesiones compatibles con coagulación intra-ratas puede afectar el normal desarrollo de la preñez y sugieren que podrían verse involucrados factores inmunológicos en el desarrollo de eclampsia
